Dear patient,

If you agree with the Terms and Agreements below please sign and date at the bottom.  ​

We are not a replacement for healthcare but here to assist in your understanding of the healthcare you have received or should receive. There is much misinformation on the internet and with artificial intelligence which is why we are healthcare navigators. We hope to provide you accurate information that will make you healthier which is our goal. 

Who is legally authorized to make your patient's healthcare decisions?

Read and initial to give your consent to be contacted. If HealthCare QB NC contacts me by telephone or email I am aware they may need to share my personal healthcare in their communication. I hereby authorize Dr. Fuller or his administrator to contact and leave messages using the following methods:​

​​                     I authorize Dr Fuller or his administrator to contact me via email, text, or phone to                         include leaving VOICEMAIL messages at the number provided by the patient. 

​                     

                     Do Not leave any messages for me by text, email, or phone EXCEPT for requests for                       me to contact Dr. Fuller.       

​​​​Process:

1) Please look at the Services we provide and either call or email us to ensure our services will work for you. 

2) Please fill out the Medical History form and submit it after we have communicated with you.

3) Please review the Privacy/disclosure information and sign stating that you understand them.

4) Book an appointment with payment before the services. Most appointments are between 60-90 minutes depending upon the complexity. We prefer they be in person but we also support phone or visual services. You have options (Packages) for a single service or monthly ongoing services.

 

By signing electrically signing this inbox below you attest that we will not prescribe medications or order labs for our patients, but help them understand the diseases, medications, lab values that they may have and advise you on other medications or modes of treatment. ​It is your responsibility to inform your primary healthcare provider of our conversations. By signing this agreement the patient understands that they are responsible for their healthcare and will HOLD Harmless HealthCare QBNC and Dr. Fuller for any healthcare events or costs that the patient should incur. ​
